Kaiser


Kaiser offers 2 plans: a traditional Health Maintenance Organization (HMO) and a High Deductible Health Plan (HDHP). Effective January 1, 2026 - there are plan modifications to Kaiser HMO



Anthem

Anthem offers three (3) plans:  two (2) Preferred Provider Organization (PPO) and one (1) High Deduction Health Plans (HDHP).  Use the Anthem Provider Finder to research available providers for this plan. Effective January 1, 2026 - Anthem PPO Select has plan modifications. 

Effective January 1, 2026- Navitus will be the new pharmacy benefit manager and plan provider for all Anthem Members

  • Navitus is replacing Express Scripts (ESI) and Anthem for prescription drug medications beginning 01/01/2026. 
  • This change will take place automatically and no action is required unless you are notified directly by Navitus regarding your personal medication. New Pharmacy ID cards will be mailed to members during the month of December.
  • You will receive a letter from Navitus in December if there is action needed from you. Navitus will provide a grace period so that action items can be completed.
